1. Call:

1-800-263-6674 between 8 a.m. and 5 p.m. Eastern Standard Time … open up your

account and place your first order at the same time

2. Provide:

• Your name, address, phone, etc.

• The name of the person who referred you:

• Sandra Maltby-Mills, ID# DH30417

3. Cost of Membership:

• Cost is $15.00 for annual membership in Canada ($19.95 in U.S.)

4. After you have set up your account and placed your first order:

• Follow up by mailing or faxing your completed “Shaklee Application”

  • Fax: 1-800-281-4160

  • Mail: Shaklee Canada Inc., 952 Century Dr., Burlington, ON L7L 5P2

• You will be sent a “Welcome Package” including your personal ID Number

5. All future orders can be done by:

• Phone 1-800-263-6674 (provide your ID number)

• Online www.shaklee.ca (login with your ID number)

Refer your friends / family members to Shaklee:

• They would follow the exact process as indicated above, with one exception ….
